Working at 3.0T with proton decoupling, Sevastianova et al. partially resolved the resonance of nicotinamide adenine dinucleotide phosphate (NADPH) from that of a-NTP. ° NADPH was higher in subjects with nonalcoholic steatohepatitis and cirrhosis than in healthy controls and correlated with disease severity. Solga et al. investigated the reliability of both and P MRS at 1.5 T in cohorts of obese diabetic subjects and healthy con-trols. ° Whereas hepatic fat could be reliably measured with relative ease, hepatic ATP proved difficult with obese subjects due to reduced P SNR. [Pg.143]

Nonalcoholic steatohepatitis (accumulation of triacylglycerol in the liver) or NASH is beginning to be identified more often in overweight individuals. This condition may progress to cirrhosis (fibrosis and loss of normal lobular structure) of the liver. In the case of T.F., measurement of serum levels of AST, ALT. and GGT were used to rule out abnormal liver function. [Pg.248]

Chemicals that damage mitochondrial structure, enzymes, or DNA synthesis can disrupt beta oxidation of hpids and oxidative energy production within hepatocytes. Prolonged interruption of beta oxidation leads to micro-vesicular steatosis which can progress to macrovesicular steatosis. This sequence of events has been noted with alcoholic and nonalcoholic steatohepatitis. The role of mitochondria has been extensively studied with nonalcoholic fatty hver disease, a major issue in human medicine. Alcoholic steatosis and other forms of hepatic steatosis have been linked to impairment of ATP homeostasis and mitochondrial abnormalities have been reported in a growing body of hterature. [Pg.556]

Nonalcoholic steatohepatitis is the most common cause of aminotransferase increases other than viral and alcoholic hepatitis. Increased transaminase concentrations have been observed in extrahepatic cholestasis, with activities tending to be higher the more chronic the obstruction. The aminotransferase activities observed in cirrhosis vary with the status of the cirrhotic process and range from the upper reference limit to four to five times higher, with an AST/ ALT ratio greater than 1. The ratio s elevation can reflect the grade of fibrosis in these patients. This appears to be attributable to a reduction of ALT production in a damaged liver. Twofold to fivefold elevations of both enzymes occur... [Pg.605]

Obesity has been recognized as a poor prognostic variable for outcomes associated with therapy for chronic HCV. Patients with a body mass index of >30 kg/m had a 77% lower chance of response to therapy compared to overweight and normal-weight patients. The exact mechanism of this interaction between weight and effect is not known. However, the natural progression of disease may be faster in obese patients and may be related to nonalcoholic steatohepatitis. ... [Pg.754]
