Nedocromil sodium

Both disodium cromoglycate and nedocromil sodium have antitussive effects in humans. In this instance, their activity occurs by increasing the depolarisation of sensory nerves, which increases the threshold for an action potential and therefore inhibits the activity of these neurons. 

Mast cell stabilizers include cromolyn sodium (Intal) and nedocromil sodium (Tilade).

In the crystalline state of a substance, the molecules are arranged in a defined unit cell that is repeated in a three-dimensional lattice [1], Since the crystal lattice can act as a diffraction grating for X-rays, the X-ray diffraction pattern of a crystal consists of a number of sharp lines or peaks, often with baseline separation. Figure 1 shows the X-ray powder diffraction pattern of the crystalline and amorphous forms of nedocromil sodium trihydrate.

Cromolyn sodium and nedocromil sodium have beneficial effects that are believed to result from stabilization of mast cell membranes. They inhibit the response to allergen challenge as well as EIB but do not cause bronchodilation.

A Cochrane Review of 20 randomised controlled trials involving 280 participants, showed that 4 mg, of nedocromil sodium inhaled 15-60 minutes prior to exercise significantly reduces the severity and duration of exercise-induced bronchus constriction in both adults and children, when compared to placebo. This effect appears to be more pronounced in people with severe exercise-induced bronchoconstriction.

Cromolyn sodium and nedocromil sodium attenuate bronchospasm induced by various stimuli, including antigen, exercise, cold dry air, and sulfur dioxide. They suppress inflammatory cell influx and chemotactic activity along with antigen-induced bronchial hyperreactivity. Also inhibited is C-flber sensory nerve activation in animal models, which may in turn suppress reflex-induced bronchospasm. 

Cromolyn sodium and nedocromil sodium are used almost exclusively for the prophylactic treatment of mild to moderate asthma and should not be used for the control of acute bronchospasm. Cromolyn sodium (disodium cromoglycate) and nedocromil sodium are stable but extremely insoluble salts (see structures below). When used as aerosols (by nebulizer or metered-dose inhaler), they effectively inhibit both antigen- and exercise-induced asthma, and chronic use (four times daily) slightly reduces the overall level of bronchial reactivity. However, these drugs have no effect on airway smooth muscle tone and are ineffective in reversing asthmatic bronchospasm they are only of value when taken prophylactically.
