The 15 mg tablet of MS Contin should be used for initial conversion for patients whose total daily requirement is expected to be less than 60 mg. The 30 mg tablet strength is recommended for patients with a daily morphine requirement of 60 to 120 mg. When the total daily dose is expected to be greater than 120 mg, the appropriate combination of tablet strength should be employed. The 30 mg tablet strength of Oramorph SR for initial conversion is recommended for patients with a daily morphine requirement of 120 mg or less.

The use of morphine via the oral route has drawbacks because of its first-pass effect however, oral morphine has been recommended for use in cancer patients for its ease of administration. In particular, the long-acting preparations of morphine, such as MS-Contin and Ora-Morph, are described as the cornerstone of pain treatment in cancer patients, either alone or in combination with nonopioids. 

Depending on the type, opioids can be injected, smoked, snorted, or taken orally. Smoking heroin is termed chasing the dragon. ControIIed-release oral tablets of morphine (MS-Contin ) or of oxycodone (OxyContin ) should never be crushed or chewed, as the entire dose of opioid released at once may be toxic.

Studies in animals and primates with these highly selective delta agonists begin to reveal that unlike mu opioid agonists such as morphine, oxy-contin, fentanyl, etc., agents acting at the delta receptor are unlikely to produce addictive liability and respiratory depression. In fact, delta agonists may actually counteract those side effects induced by mu opioids.

In clinical practice various oral dosage forms are also used for rectal administration. There is often little scientific support for such use, nevertheless it may be effective. Evidence exists for rectal administration of an oral controlled release morphine tablet (MS-Contin ) [2, 3]. Rectal use of temazepam oral capsules is not effective. There is a broad variation in time to reach the maximum plasma concentration and in the biological availability. Better outcomes are obtained with temazepam dissolved in glycofuroFethanol water = 5 1 4 and given rectally as an enema [4]. 

Morphine sulphate in a matrix of a fatty base (Witepsol W25) with colloidal anhydrous silica (Aerosil R 972 ) and hydroxypropylmethylceUulose (HPMC 4000) [10] showed an absorption rate and a biological availability equivalent to orally given morphine sulphate retard tablets (MS Contin ). Such suppositories can be made as a regular pharmacy preparation [11] but in our opinion this requires more convincing evidence that morphine release is sufficiently reproducible. MS-Contm suppositories (not available in all countries) have a fatty base with sodium alginate and calcium phosphate [12].
