Patterns of mesenchymal differentiation in cutaneous neoplasms are no less diverse than those encountered in deeper soft tissues. Fibroblastic or myofibroblastic, fibrohistiocytic, muscular, neural, epithelial, and vascular lesions may be seen as primary tumors in the dermis and subcutis. As is also true in deep soft tissues, the histologic evaluation of those neoplasms may fail to provide an unequivocal diagnosis. Hence immunophenotyping has proven to be valuable in this context. The diagnostic separation of various spindle-cell, polygonal-cell, epithelioid-cell, and small-cell lesions of the skin is assisted in many settings by immunohistochemical analysis. [Pg.479]

It is likely that MFH is not a unified entity in the skin or elsewhere, but instead represents a histologic pattern that is a common final pathway of differentiation for several modes of mesenchymal neoplasia. That concept was first espoused by Brooks. Thus an MFH-like pattern may be seen in conjunction with other mesenchymal images in the same tumor mass, as a consequence of clonal evolution. An unqualified diagnosis of MFH can be made immunohistologically only if one is dealing with a pleomorphic malignant tumor that is vimentin reactive and lacks epithelial, myogenous, neural, and endothelial markers. At present, there are no proactive determinants that define this neoplasm. [Pg.481]

Hyaluronic acid is found in varying amounts, up to 5 % of the dry weight, in different animal tissues, the highest concentrations being in umbilical cord, vitreous humor, joint fluids, and skin. It has also been reported in cornea, bone, heart valve, tumor fluids, in the cock s comb, in the chick oviduct, and in groups A and C hemolytic streptococci.
