Historically, the use of xanthines has been hampered by poor aqueous solubiUty, rapid but highly variable metaboHsm, and the existance of a low therapeutic index. SolubiUty problems were partially solved by the preparation of various salt forms, eg, aminophylline. However, it was since recognized that the added base in aminophylline only increases solubiUty by increasing pH and thus does not affect the rate of absorption from the gut (65). Thus, in more recent medical practice, theophylline is commonly dispensed in anhydrous form and aminophylline is only recommended for iv adrninistration.

We have developed a system based on SNOW-MED to extract medical information from herbal texts. SNOW-MED is a semantic index that recognizes relationships between groups of words [26], For example, the semantic map for thrush is related to yeast, infection, and microbe. Although this system may eventually allow a potential pharmacological function to be extrapolated, we are currently using the system to simply extract disorders from the text. We have used the Mayo Vocabulary Server to perform this data mining [34, 35]. 

Is the patient hemodynamically stable If not, admit to the intensive care unit for ongoing treatment and monitoring. A PA catheter (or CVP catheter) should be inserted by trained medical personnel. Monitor PAOP to a goal pressure of 14 to 18 mm Hg and minimum cardiac index of 2.2 L/minute per square meter (alternatively CVP 8 to 15 cm H20). 

Theophylline is a non-specific phosphodiesterase inhibitor that increases intracellular cAMP within airway smooth muscle resulting in bronchodilation. It has a modest bronchodila-tor effect in patients with COPD, and its use is limited due to a narrow therapeutic index, multiple drug interactions, and adverse effects. Theophylline should be reserved for patients who cannot use inhaled medications or who remain symptomatic despite appropriate use of inhaled bronchodilators.

A high index of suspicion should be maintained for patients at risk for CNS infections. Prompt recognition and diagnosis are essential so that antimicrobial therapy can be initiated as quickly as possible. Generally, when treating a sexually transmitted infection (STI), the index patient should be provided with a sufficient quantity of medication for the partner to be treated, increasing the probability that the initial infection will be cured in both patients. 

Notice that treatment is randomly assigned within the given blocks and that there are two placebos and two study medications in each block. Also notice the index variable. The order of the randomization scheme is critical to the usefulness of the scheme, as that is the order in which patients are assigned treatment. If the order of the scheme is altered in any way, then the scheme is damaged.

IPA covers approximately 700 worldwide pharmaceutical, medical, herbal, cosmetics, and health-related publications. IPA features all abstracts from American Society of Health-System Pharmacist s Annual, Midyear Clinical, and Home, Hospice, and Long-Term Care Meetings, coverage of state pharmacy journals, and American Pharmaceutical Association and American College of Clinical Pharmacy meeting abstracts. IPA indicates articles that offer CE credit. IPA is commercially available in several formats.

Method The authors use information on all non-hospital sales of pharmaceutical products in 1992 in a sample of countries consisting of the USA, Canada, Germany, France, Italy, Japan and the UK. The database was provided by Intercontinental Medical Systems (IMS). The empirical analysis is based on the calculation of the Paasche and Laspeyres price indexes and the ratio between them. The descriptive analysis is completed with the econometric analysis (quasi-hedonic model) of the determining factors of the variation in the relative prices of each active ingredient in each country taking the USA as the point of comparison.
