The relocation of treatment from chronic hospital settings to outpatient community mental health centers is, in great part, due to the efficacy of antipsychotics. Naturalistic studies before the era of psychotropics revealed that two of three psychotic patients (primarily schizophrenic) spent most of their lives in state asylums. Before the mid-1950 s, there had been a steady increase in state hospital populations, which paralleled the general population growth, but after the introduction of antipsychotics, there was a marked reduction in those hospitalized for various psychoses. Presently, more than 95% of these patients live outside of the hospital, even though many continue to relapse or demonstrate residual symptoms. Thus, although the antipsychotics have not been a panacea, they make community-based care a reality for many who would otherwise have remained chronically institutionalized.

Public concern over the deficiencies of current systems of care for the mentally ill grew in the 1950s and culminated in the landmark Community Mental Health Centers Act of 1963, which emphasized services in the community. Prevention of mental illness through consultation and education was one of the ultimate goals, and schools were seen as ideal settings in which to implement these aims (Caplan, 1970). 

The efforts of community mental health centers (cmhcs) to reach out to their communities, while not always successful, helped pave the way for later school-based and school-linked mental health centers (Flaherty, Weist, Warner, 1996). Furthermore, their concern about having therapists from the community who could speak the language of their clients as well as the use of paraprofessionals foreshadowed today s notions of cultural competence.
