The authors of the Guidelines for Risk Based Process Safety (KBPS) (CCPS, 2007a) described the need for constant vigilance as the price of maintaining an effective process safety management system. An operator not only must be vigilant (aware of both past and current performance), but must not assume that current performance will be maintained, much less improved, without intentional evaluation of critical parts of systems and their performance. Performance measurement and metrics are a critical part of the RBPS system. 

Table 7-3 contains commonly used conversion factors for household measurement and metric measurement. 

All doses are calculated using the metric system. You must be able to convert within the metric system and convert between household measurements and metric because patients are likely to self medicate using household measurement—such as a teaspoon— rather than using metric measurements. 

Outcomes and Feedback Whatever type of supplier partnership is implemented, the effectiveness of the relationship must be evaluated and possibly adjusted. The key to effective measurement and feedback is how well the drivers of partnership were developed at the outset. At this beginning point, the measurement and metrics of relating to each driver should have been made explicit. These explicit measures then become the standard in evaluation of the partnership outcomes. Feedback can loop back to any step in the model. Feedback can take the form of periodic updating of the status of the drivers, facihtators, and components.

Some of the elements in Table 1.4, such as Management of Change, are identical to those in Table 1.3. Others are modified—e.g.. Prestartup Safety Review becomes Operational Readiness. But some of the elements in Table 1.4, such as Measurements and Metrics, are completely new. One of the topics in the original OSHA list— Trade Secrets— has been removed. 

An analogy can be drawn between financial audits and economic analysis. A financial audit checks that a company is conforming to the goals and standards set by external agencies such as the tax authorities and the company s own finance department. Economic analysis, which corresponds to measurement and metrics, seeks to understand issues such as the best pricing level for the company s product, labor rates, and how to finance the enterprise. 

Measurement and metrics for software maintenance is a difficult topic. There are a large number of product and process metrics for software. Product metrics tend to concentrate on complexity type metrics and there have been some attempts at correlating some of these metrics with maintenance effort. One of the problems here is there is no definitive and universal definition of maintainability. Another problem is that organizations are reluctant to publish data on their systems thus making any comparisons difficult.
