Loperamide

Loperamide. 4-(4-Chlorophenyl)-4-hydroxy-A7,A/-dimethyl- 7, 7-diphenyl-l piperidinebutanamide monohydrochloride [43552-83-5] (Imodium) (14) is practically iasoluble ia water (0.002%) at physiological pH. Its crystals are not affected by light and it is not hygroscopic. It may be synthesized (12). [Pg.203]

Loperamide is similar ia action and use to diphenoxylate however, it does not need to be formulated with atropiae and is available by prescription and OTC. It is reported to have fewer central nervous system side effects than diphenoxylate. [Pg.203]

Loperamide stimulates enteric nervous system receptors, inhibiting peristalsis and fluid secretion. It improves stool consistency and reduces the number of stools.21 Consequently, it is most useful in patients who have diarrhea as a prominent symptom. Bowel symptoms in MS patients can include both fecal incontinence and constipation. Fecal incontinence is difficult to treat. Diarrhea may occur from effects of chemotherapy on the lower portion of the GI tract. Diarrhea can be severe and may need to be treated with intravenous fluids and electrolytes. Infectious causes, such as C. difficile, should be ruled out.
