Some States adopt two levels of classification/load resistance depending upon a deterministic frequent/infrequent categorization infrequent faults (<10 per year) require one line of protection, whereas frequent faults (>10 per year) require two lines of protection. 

It is considered that the concept of error-free software cannot currently be realised in any but the simplest of programs and hence measures to reduce the possible effects of software errors are taken. The purpose of the OPT is to document the implementation of essential LOPs to ensure successful prevention, control, or mitigation of phenomena that could potentially damage the nuclear system. 

While PPE (Figure 2-3) seems to be the most common form of protection and in some cases is mandated ly law, it should be considered a last line of protection and always used in conjunction with engineered and administrative controls. PPE may also be appropriate for controlling hazards while engineered and work practice controls are being installed. 

Personal protection measures are the first line of protection against vectors of diseases. Although personal protection measures can reduce the incidence of vector-borne disease, they can rarely eliminate the risk because not all individuals in a community use the measures. In this chapter, the strategies used to reduce nuisance and the potential reduction in the transmission of diseases are discussed. Readers will note that none of the methods or combination of methods is 100% effective because of inconsistency of repellent application, and not aU individnals in a community adhere to personal protection.
