Laparoscopy

Article of Manufacture. An article of manufacture is an iavention such as a two-headed tooth bmsh, an iatravenous fluid bag, or an optical fiber "made" by a machine. One example of an iavention which could be considered an article of manufacture is U.S. Patent No. 5,241,990 tided "Irrigation/Aspiration Valve and Probe for Laparoscopy" (2).

The most disturbing adverse reaction to clofazimine is a red-brown discoloration of the skin, especially in light-skinned persons. A rare but serious adverse reaction is acute abdominal pain significant enough to warrant exploratory laparotomy or laparoscopy. Other infrequent side effects include splenic infarction, bowel obstruction, paralytic ileus, and upper GI bleeding.

A 54-year-old woman and a 62-year-old man with catheter blockages both developed aseptic peritonitis (166). In both cases clots had earlier been removed by laparoscopy, which also showed diffuse thickening around the tip and in the peritoneal fat, with fluid accumulation or inflammation and whitish urticarialike plaques. The tissue was granulomatous, with histiocytes, fibrosis, and pseudo-amyloid material that could not be labeled by anti-insulin antibodies. The peritoneal fluid contained a lot of fibrin, monocytes, lymphocytes, and macrophages, but no bacteria or cancer cells.

In the majority of patients, this cancer remains occult and becomes symptomatic after it has already metastasized to the peritoneal cavity. At this stage, it usually presents with malignant ascites. It is important to accurately stage this cancer with laparoscopy, ultrasound, and CT scanning. Patients with stage I disease appear to benefit from whole-abdomen radiotherapy and may receive additional benefit from combination chemotherapy with cisplatin and cyclophosphamide. 

A laparoscopy is performed and the cysts drained. The patient is prescribed GnRh analogue plus add-back therapy. Two months later she is readmitted with left loin pain, hot, cold and dizzy symptoms. The impression is a flare-up of the endometriosis. The pain team prescribed morphine 2 hourly. Patient had radical operation subtotal abdominal hysterectomy and bilateral salpingo-oophorectomy.
