Iron clearing efficiency is a measure of how much iron excretion is promoted by a chelator. It is a comparison of actual iron excretion versus theoretical iron clearance this number is given as a percentage. For example, because DFO forms a very tight 1 1 complex with Fe(III), one millimole of DFO (636 mg as its mesylate salt) given sc to a patient should cause the excretion of one milligram-atom of Fe(III) (56 mg). Unfortunately, the efficiency of DFO in clinical use is only about 5 to 7% (25, 37). [Pg.369]

The animal model selected for this series of experiments was the iron-overloaded C. apella monkey. In this model, we are able to measure both total iron excretion, expressed as iron clearing efficiency, and the modes of iron clearance, that is, biliary and renal. Typically, dry potassium nitrate is pulverized in a ball mill. Sulfur is milled into cellular charcoal to form a uniform mix in a separate ball mill. The nitrate and the sulfur—charcoal mix are screened and then loosely mixed by hand or in a tumbling machine. Magnetic separators may be used to ensure the absence of ferrous metals. The preliminary mix is transferred to an edge-mimer wheel mill with large, heavy cast iron wheels. A clearance between the pan and the wheels is required for safety purposes. The size of this gap also contributes to the density of the black powder granules obtained. Water is added to minimize dusting and improve incorporation of the nitrate into the charcoal. BROWN et al.i95) have given data on the performance of 1.5 m diameter sulphonators and nitrators of 3.4 m3 capacity as used in the dyestuffs industry. The sulphonators were of cast iron and had a wall thickness of 25.4 mm the annular space in the jacket being also 25.4 mm. The agitator of the sulphonator was of the anchor type with a 127 mm clearance at the walls and was driven at 0.67 Hz. In humans the clearance rate of Hb is higher than that of HpHb (L4, Lll). Murray et al. (M6) found this also to hold for rabbits and, by studying the elimination in nephrectomized animals, they also proved that the difference was not due to urinary loss of Hb. In terms of pharmacokinetics, LVX has an excellent profile. With an oral dose of 500 mg, LVX has a bioavailability of >99%, an AUC range of 41.9-47.7 mgh/mL, Cmax of 4.5-6.2 mg/mL, a clearance of 10.5-11.9 L/h, and a volume of distribution (Vd) of 1.3 L/kg (Hurst et al., 2002). In terms of protein bound material, only 24-38% is affected (Fish, 2003). Like other fluoroquinolones, there is a 19-44% AUC reduction when co-administered with an aluminum or magnesium antacid or iron sulfate (Qaqish and Polk, 2003). All quinolones interact with multivalent cations, forming chelation complexes resulting in reduced absorption. Major offenders are antacids vitamins containing calcium and iron can also be problematic.
