Insulin glargine is given SC once daily at bedtime This type of insulin is used in the treatment of adults and children with type 1 diabetes mellitus and in adults widi type 2 diabetes who need long-acting insulin for die control of hyperglycemia. [Pg.493]

Do not mix or dilute insulin glargine with any other insulin or solution because glucose control will be lost and the insulin will not be effective. [Pg.494]

The basal insulin component may be provided by once- or twice-daily NPH or detemir, or once-daily insulin glargine. Most type 1 DM patients require two injections of all insulins except insulin glargine. All of the insulins (with the exception of insulin glargine) have some degree of peak effect that must be considered in planning meals and activity. Insulin glargine or insulin detemir is a feasible basal insulin supplement for most patients. [Pg.235]

An apparently more effective method for prolonging the half-life of insulin in the blood is to add substituents at the end of the A- or B-chain (or both) that alter the chemical properties of the molecule and delay its breakdown in the body. A product known as HOE 901 (insulin glargine) has two glycine residues added to one end of the B-chain and the A21 asparagine residue replaced with another glycine residue. These changes modify the acidity of the insulin molecule, reducing the rate at which it is absorbed and metabolized in the body. [Pg.69]

Insulin glargine Once-daily subcutaneous administration for the control of hyperglycemia. [Pg.292]

INSULIN GLARGINE Give insulin glargine subcutaneously once daily at the same time every day. The dose may be administered at any time during the day. IV administration of the usual subcutaneous dose could result in severe hypoglycemia. [Pg.293]

Insulin glargine must not be diluted or mixed with any other insulin or solution. Only use if clear and colorless with no particles visible. [Pg.293]

Children - Insulin glargine can be safely administered to children 6 years of age and older. Administration to children younger than 6 years of age has not been studied. The dose recommendation for changeover to insulin glargine is the same as described for adults. [Pg.293]

Initial dosing - In a clinical study with insulin-naive patients treated with oral antidiabetic drugs, insulin glargine was started at an average dose of 10 units once daily, and adjusted to a total daily dose ranging from 2 to 100 units. [Pg.293]

No pronounced peak small amounts of insulin glargine are slowly released, resulting in a relatively constant concentration/time profile over 24 hours. [Pg.296]

Pregnancy Category 6 Category C (insulin glargine, insulin aspart, insulin glulisine). [Pg.298]

Lactation Lactating women may require adjustments in insulin dose and diet. It is unknown whether insulin glargine, insulin aspart, or insulin glulisine are excreted in significant amounts in breast milk. [Pg.298]

Insulin glargine - Safety and efficacy of insulin glargine have been established in children 6 to 15 years of age with type 1 diabetes. [Pg.298]
