More recently, large databases have been used to estimate the effect of drug co-payment in the USA under different insurance schemes.10 The conclusion reached is that there is a significant interaction effect between the behaviour of demand and prescriber incentives. Thus, larger prescription drug copayments are associated with lower expenditure when the doctor does not share the financial risk of the cost of the drugs (that is, practises in an independent practice association) but this effect is barely perceived in managed care models in which the doctor has incentives for cost containment. 

A network model HMO is a health plan that contracts with many large physician groups and community pharmacies to provide care to its members. As with group model HMOs, network HMOs do not own their own facilities and typically compensate each provider group at a negotiated, capitated rate. Finally, an individual practice association (IPA) is an HMO model that contracts with independent physicians, pharmacies and providers in their own practice settings to provide medical services to its enrollees. 

An exception to this rule would relate to a self-determination entity that decides to associate, but not integrate, with another state, rather than opting for independence. Through association, the self-determination status of the entity is retained, or one might say it is transformed into a case of constitutional self-determination. However, there is very little practice of this kind.

In principle, Chen, given the flux relations there is no difficulty in constructing differencial equations to describe the behavior of a catalyst pellet in steady or unsteady states. In practice, however, this simple procedure is obstructed by the implicit nature of the flux relations, since an explicit solution of usefully compact form is obtainable only for binary mixtures- In steady states this impasse is avoided by using certain, relations between Che flux vectors which are associated with the stoichiometry of Che chemical reaction or reactions taking place in the pellet, and the major part of Chapter 11 is concerned with the derivation, application and limitations of these stoichiometric relations. The usual practice in these appHcations is to concentrate on model development and computation rather than on statistical aspects. In general, nonlinear regression should be appHed only to problems in which there is a weU-defined, clear association between the independent and dependent variables. The generalization of statistics to the associated confidence intervals for nonlinear coefficients is not well developed.

Note that the NAOs, NBOs, and associated occupancies are in principle uniquely determined by y, and thus by itself.
