Monogenic dyslipoproteinemias can generally be grouped into five categories (1) hypertriglyceridemia with an increase in chylomicrons and the clinical sign of pancreatitis, (2) mixed hyperlipidemia with an increase in chylomicron and VLDL remnants and an increased risk of premature atherosclerosis, (3) hypercholesterolemia with an increase in LDL and an increased risk for premature atherosclerosis, (4) hypoalphalipoproteinemia with low HLD and an increased risk for premature atherosclerosis, and (5) hypolipoproteinemia with a decrease in VLDL and LDL, which may lead to neurological disease. 

The most striking consequence of treatment with sirolimus is dose-dependent hyperlipidemia with significant increases in both cholesterol and triglyceride serum concentrations, which resolve after dosage reduction or sirolimus withdrawal (1065).

Current guidelines do not advocate carnitine administration for the treatment of hypertriglyceridemia. Studies of carnitine for this indication have varied widely in duration and have used both oral and intravenous administration in varying doses (1 mg/kg to 2 g/day intravenously, 10 mg/kg per day to 3 g/day orally). It has been suggested that this is due to the inhibition of lipid-regulating proteins which have active sites with structural homology to that of HIV protease. The syndrome includes hyperglycemia and insulin resistance or hyperlipidemia, with altered body fat distribution. Buffalo hump, gynecomastia, and truncal obesity may occur with facial and peripheral lipodystrophy. The syndrome has been observed with protease inhibitors used in HAART regimens, with an incidence of 30-50% and a median onset time of approximately 1 year s duration of treatment.
