Hyoscine hydrobromide

Motilium contains domperidone, v/hich is a dopamine antagonist and acts on the chemoreceptor trigger zone. It is ineffective in motion sickness. Stugeron contains cinnarizine Avomine and Phenergan contain promethazine and Kwells contains hyoscine hydrobromide. Cinnarizine and promethazine are antihistamines, which are indicated in motion sickness and hyoscine hydrobromide is an antimuscarinic agent that is also used in motion sickness. 

Calculate the percentage of the stated content of hyoscine hydrobromide in travel sickness tablets from the following data. The assay is carried out in a manner similar to the eyedrop assay described above. In one study hyoscine premedication had detrimental effects on memory and on motor tasks compared with placebo, while atropine did not (309), although the difference is unlikely to be absolute. In view of certain of these effects, hyoscine hydrobromide is not a suitable antiemetic for those likely to drive vehicles before the effect has worn off, for example air passengers.

Hyoscine hydrobromide is used primarily in motion sickness doses of 0.6 mg appear to be effective, particularly in combination with drugs having a mild central stimulant effect such as ephedrine. 

Despite being a stereoisomer of atropine, hyoscine tends to have central depressant effects in conditions where atropine might be expected to cause excitation however, excitation can occur (1). Hyoscine hydrobromide produces somnolence and dryness of the mouth in a high proportion of patients, and when ephedrine is not given the somnolence is likely to be present in the majority some individuals also have headache, giddiness, and blurred vision. 

In 23 patients who took an overdose of Sleep-Qik (valerian dry extract 75 mg, mean dose 2.5 g, hyoscine hydrobromide 0.25 mg, and cyproheptadine hydrochloride 2 mg) the main problems were central nervous system depression and anticholinergic poisoning (7). There was no evidence of liver damage, subclinical, acute, or delayed. It is unlikely that valerian contributed to the adverse effects in these cases. 

Subsequently, Chan reported on twenty-four cases of overdose of a product containing valerian dry extract 75 mg, hyoscine hydrobromide 0.25 mg, and cyproheptadine hydrochloride 2 mg. Six patients developed vomiting, and fifteen underwent gastric lavage. Co-ingestants included alcohol (n = 10), cold products n = 3), hypnotics (n = 2), unknown drugs (n = 2), and gasoline (n = 1). The drugs used are listed in Table I. Atropine sulphate was purchased from B.D.H. Ltd and hyoscine hydrobromide from Macfarlan Smith Ltd. All the other drugs were prepared in these laboratories and were used as the hydrochlorides. Doses were given in mg/kg as indicated above but for calculation of correlation coefficients these were converted into /xmoles/kg. 

The drugs were dissolved in sterile physiological saline and administered by the intramuscular route. The drugs and doses used in this study were atropine sulphate (2.4 mg/kg), hyoscine hydrobromide (0.24 mg/kg), N-methyl-3-piperidyl benzilate hydrochloride (0.5 mg/kg), LSD-25 tartrate (10 jug/kg) and chloropromazine hydrochloride (5 mg/kg).
