A child had anaphylactic shock when he received a second course of basiliximab at the time of a second renal transplantation (6). There were antibasiliximab IgE antibodies in the serum, but no IgE reactivity toward a control murine IgG2a monoclonal antibody, suggesting that the IgE response was directed exclusively against basiliximab idiotypes. Daclizumab, a humanized monoclonal antibody, is composed of only 10% murine antibody sequences and therefore is less immunogenic. These findings suggest that despite the similar compositions of human and mouse antibody protein sequences the IgE responsiveness is significantly different.

In the context of cytokine regulation of IgE responses it is worth noting that other molecules may play direct or indirect roles that are possibly of lesser importance. It has been shown, for instance, that interleukin 8 inhibits selectively the synthesis of IgE by IL-4 primed human B lymphocytes through a mechanism that is independent of the action of IFN-7(Kimata et al 1992). In addition, recent evidence suggests that interleukin 7 (IL-7), a cytokine produced by stromal cells in the thymus and bone marrow, may indirectly influence IgE responses secondary to differential effects on the production of IL-4 and IFN-y It was observed that IL-7 caused a preferential upregulation of ILN-y expression by activated human T-lymphocytes (Borger et al., 1996).

Extensive effort is ongoing to develop further and characterize the intranasal model. This includes continued mechanistic investigations on the IgE response potential of the model, characterization of respiratory and inflammatory pathophysiology, responsiveness to enzymes delivered in different product matrices, and responses to individual enzymes when delivered in combination with other enzymes. As these results are forthcoming, they will be compared with prior results obtained from GPIT studies and with human experience in our efforts to validate the model fully and establish it as an appropriate and useful procedure for prediction of the respiratory hypersensitivity potential of new enzymes. 

Omalizumab is a monoclonal antibody that selectively binds to human IgE, inhibiting the binding of IgE to the high-affinity IgE receptor on the surface of mast cells and basophils, and limiting the degree of release of mediators of the allergic response. It is indicated in the treatment of moderate to severe persistent asthma in patients who have a positive skin test or in vitro reactivity to a perennial aero-allergen and whose symptoms are inadequately controlled with inhaled corticosteroids.
