Emergency procedures will be called upon following any abnormal occurrence such as an explosion, fire, release of unburned gas or production and release of toxic products of combustion. It is essential to carry out an emergency shutdown procedure in a safe way that prevents the hazard continuing. 

A broad spectrum of diseases in children are known (or suspected) to be associated with unhealthy environments. For much of the world, traditional environmental health hazards continue to remain the primary source of ill-health. These include lack of adequate nutrition, poor sanitation, contaminated water, rampant disease vectors (e.g. mosquitoes and malaria), and unsafe waste disposal. In addition, rapid globalization and industrialization coupled with unsustainable patterns of production and consumption have released large quantities of chemical substances into the environment. Although the term environmental exposure can encompass a variety of factors, the focus of this document is specifically on environmental chemical exposures. Most of these substances have not been assessed for potential toxicity to children, nor have the most vulnerable subpopulations of children been identified. The incidence of a number of important paediatric diseases and disorders (e.g. asthma, neurobehavioural impairment) is increasing in several parts of the world. Although a variety of factors are likely to be involved, this may be due, in part, to the quality of the environment in which children live, grow, and play.

Obvious safety hazards continue to slip through the net on too many projects, suggesting too few audits, auditing too late, inexperienced auditors or project managers struggling with how to respond to audit reports. 

If certain hazards continue to occur after being corrected, the management systems are not performing properly. Someone is not taking responsibility for correcting the identified hazards. If this is the case, responsibilities may not be clearly defined or those who are responsible may not be held accountable [7]. 

As mentioned previously, PIMs also offer vast improvements in terms of health, safety and environmental concerns for process operations compared to the other t)q)es of membranes and conventional SX. Handling fabricated PIMs is effectively as safe as handling other plastic products. The discrete and robust nature of PIMs also allows for off-site fabrication this means that the in-situ organic solvent hazard can be completely removed when PIMs are used. Chemical exposure and its associated hazards continue to be more stringently regulated with time so PIMs may present an opportunity to reduce compliance costs. 

Problems in the hazard control system are identified when the same type(s) of hazard continue to occur after an action plan is completed and verified.
