Hazard tracking log

The hazard tracking log provides a means of tracking hazards throughout the life cycle and documenting the status of corrective actions. 

The hazard tracking log is prepared as an add-on to the PHA and prepared as soon as the PHA is completed and delivered. The hazard tracking log is then updated as control actions are taken and verified. Summary reports of hazard tracking status are prepared and available for all formal system safety reviews. 

The hazard tracking log is normally prepared and maintained by the safety staff.

Hazard Tracking Log. In addition to the above analysis, a hazard tracking log (HTL) should be maintained. This log is to ensure all open loops are closed and ensures the appropriate level of management is identified as being involved in the acceptance of risk. 

A systematic method must be established to document and track each identified hazard throughout the life cycle and to ensure that control measures are, in fact, implemented. The method recommended is to use a hazard tracking log as a permanent part of the project documentation. 

Because the hazard tracking log is an extension of the preliminary hazard analysis, the PHA must be complete before the hazard tracking log is needed. Information needed to complete the hazard tracking log includes appropriate drawing numbers, change orders, and other documents that verify that hazard controls have been implemented. 

Figure 8-3 shows a hazard tracking log incorporated in a preliminary hazard analysis worksheet. 

Reference Document column Lists the drawing and/or other hazard tracking reference that documents the corrective action(s). Verification Signature(s) column Contains the signature of the person(s) responsible for maintaining the hazard tracking log and the date the verification was made. 

Detailed project description and detailed accident information are required to conduct an in-depth accident analysis. Historical data, existing hazard analyses, and the hazard tracking log are also important.
