Hazard analysis proposed methodology

If a recommendation asks for a change in the process, the action must undergo a formal process hazard analysis (PHA) study, such as a HAZOP or other methodology, before implementation. This systematic and formal approach identifies and evaluates hazards associated with the proposed revisions. The study may uncover failure scenarios, adverse consequences, and obscure relationships that are not immediately apparent. The CCPS publication Hazard Evaluation Procedures i is an excellent guide to selection and proper application of PHA methodologies. [Pg.314]

Chemical Hazard Analysis (CHA)— A CHA is derived from HAZOP methodologies and can be considered a precursor to a PHA. It is applicable to analyzing petrochemical or chemical processing hazards. The same seven basic HAZOP guidewords are used no, more, less, part of, reverse, as well as, and other. While CHA assumes the proposed chemical reaction is basically safe when conducted as specified (should... [Pg.141]

In support of the framework, the author analysed the hazard as a system concept and developed existing methodologies further, identified and defined the attributes, and their hierarchical structure, of a Hazard as a system. The author also analysed the concept of the safety case as the inquiry model identifying a number of constraints and requirements that a safety case must satisfy. Subsequent to the analysis of these, the author proposed the general high level approach to the structure of the safety case as a fact net, reasoning model. [Pg.192]
