Good intentions

Since the resources for data collection systems will be provided by senior management it is essential that information from the system is fed back to policy makers at this level. It is also important that the system indicates the problem areas as well as the successes. Many organizations have drifted to a state where safety standards have fallen to below acceptable levels over time as a result of suppression of information feedback to senior managers. This may be carried out with good intentions, but its long-term effect can be disastrous.

As can be seen, the real cause of most accidents is what might be classified as human errors. Most people have good intentions to perform a function properly, but where shortcuts, easier methods or considerable economic gain opportunities appear or present themselves, human vulnerability usually succumbs to the temptation. Therefore it is prudent in any organization, especially where high risk facilities are operated, to have a system in place to conduct considerable independent checks, inspections, and safety audits of the design and construction of the installation. 

There is, as always, a need for good quality data. Most of this is now available in electronic form and Chapter 11 lists some of the databases available. In spite of proclaimed good intentions, there is little systematic documentation of the successful application of plastics and their lifetimes, only examples of unexpected failure. There is a need for medium-term, lightly accelerated tests under intermediate conditions to validate the predictive models. While inspection of components at end-of-life is more prevalent than expected, there is a need for coupling it to predictive techniques to validate these techniques and to close the loop of life prediction.

The fundamental concept of chemical kinetics is that of reaction mechanism. In the broad sense, the word mechanism ("detailed , "intimate ) is the comprehensive interpretation of all experimental data accumulated on the complex reaction process. In this mechanism, one should discriminate individual stages and reaction steps, give characteristics for intermediates, describe transition states of individual steps, provide energy levels of substances, etc. As far as catalytic reactions are concerned, one should characterize surface properties, examine the adsorption character, etc. "I want to know everything about a complex chemical reaction this is the way one must understand chemists when they speak about their intention to investigate a detailed mechanism. Whether it is possible to realize such good intentions at a modern theoretical and experimental level will be another question. 

The first series of modifications featured in this chapter were all motivated by Good Intentions. In spite of creative ideas and considerable effort, these modifications failed because no one took the time to examine and expose their weaknesses. These undetected weaknesses caused undesired side effects.

There were good intentions when a small chemical terminal in Savannah, Georgia, that supplied the paper and pulp industry decided to change their product mix. Their terminal was to be converted from handling all nonflammable chemicals to storing large quantities of flammable crude sulfate turpentine (CST).
