A large amount of sputum is a characteristic manifestation of DPB. Sputum volume markedly decreases after EM therapy. The reduction of sputum volume is the most sensitive parameter as shown by the double-blind study mentioned here. Goswami et al. [11] first reported that EM dose-dependently inhibited mucous secretion from airway mucosa with a marker of glycoconjugate in vitro. Tamaoki et al. [12] first observed in vitro ion transport on epithelial cells and reported that EM dose-dependently inhibited ion transport when erythromycin attached to the serosa. Furthermore, they found this inhibition was due to blocking of the chloride channel. The inhibition of the chloride channel also means that erythromycin inhibits the secretion of water that moves with the chloride ion. Inhibition of mucus and water secretion from the epithelial cells can be interpreted as an important mechanism of action in improving hypersecretion in patients with DPB. [Pg.537]
