Family dynamics

Family dynamics can change radically when one member has a sickness and disability and needs a lot of care. It was difficult for all of us. My relationship with them had been one of rebellion and defiance, typical for a precocious teenager in the 60s and a 20-year-old in the 70s. I didn t want to share their house or their values, but there I was, completely dependent on them again, and they were pretty much stuck with me in this condition. There was nothing to do but submit to this child role. I didn t know if I d get well or not, but I felt stronger after a couple of years, and that had to be good enough. I stayed with my parents for almost three years. 

Within any of these treatment settings, any of several specific psychiatric treatments can be applied. A comprehensive treatment plan that incorporates both medications, if indicated, and psychosocial treatments is generally believed to be most effective. The treatment plan should be customized to meet the patient s particular needs. Treatment planning should take into account not only the primary substance that is being abused but also family dynamics, the physical and social impact of the substance use disorder, and the presence of any complicating medical or psychiatric illnesses. Psychosocial treatments encompass several formats such as individual psychotherapy, support groups, and behavioral modification. 

The centre stage occupied by the drug problem in these households was both cause and justification for skewed family dynamics it was the fuel of family problems and also a resource. In providing a ready-made explanation for any family problem, it justified a cycle of negative reaction that progressively locked the family member with the drug problem into the position of family scapegoat. 

All of the practitioners referred to the complex and very often turbulent family histories that many of their clients reported. Children with other chronic medical conditions, such as asthma [37] or atopy [38] and cystic fibrosis, can be prone to sleep disruption either from medication used to treat the underlying condition, or as a result of poor symptom control. The family dynamics may be viewed differently by mother and daughter. Bulimic daughters are more inclined to have a negative perception of family function than their mothers (Bonne et al. 2003). Teasing by siblings, especially brothers can also add to tensions within the family and further promote the eating disturbance. 

A family counseling component in treatment. Because family relationships have been strained by the young person s substance use and the traditional parental response to it, these relationships need to be repaired. Unaltered family dynamics risk setting the child back when he or she returns home from treatment.

Non-response to treatment in OCD is associated with substantial impairment and is defined as poor or non-response to two or more trials of SSRIs. However, a trial of clomipramine up to 200-300 mg/day for at least 10 weeks should always be considered before viewing a patient as refractory to treatment. Disconnection and rejection - the individual has an expectation that their needs for security, safety, stability, nurturance, empathy, acceptance and respect will not be met in a consistent manner. The typical early family dynamics involve cold, rejecting, withholding, lonely, explosive, unpredictable or abusive behaviour from others. The EMS in this domain are abandonment/instability (perceived instability or unreliability of personal support), mistrust/abuse (expectation that others will hurt, abuse, humiliate, lie, manipulate, cheat, etc.), emotional deprivation (perception that one s needs for emotional support will not be met), defectiveness/ shame (the feeling that one is defective, bad, unwanted, inferior, flawed or invalid in some important respect) and social isolation/alienation (the feeling that one is different from others and isolated from any social group). 

Impaired autonomy and performance - the individual does not perceive that they are able to function independently, or perform successfully in any way, leading to feelings of inadequacy. Typical family dynamics involve overprotective parenting, enmeshment and failure to support independent activity, all undermining the individual s self-confidence. The EMS in this domain are dependence/incompetence (unable to perform tasks competently without substantial help), vulnerability...
