Environmental sample audit

To assist you in the development of an environmental compliance audit program, a sample environmental compliance and environmental management audit checklist is included in Appendix B of this chapter. The checklist has been developed to cover the eleven subject compliance areas discussed earlier. To maximize the use of the model checklist, each semiconductor facility audit team should tailor it to reflect the scope of their manufacturing activities, specific site conditions, chemical usage patterns, and inclusion of applicable state, regional and local regulatory requirements.

The contract can specify the amount of time the client needs to remove its materials from the toller s facility and lays out notification procedures for dissolving the contract. Chapter 6, Closure and Audit, Example 6-1 is a sample contract termination checklist. It is an illustration of one company s inventoiy list for closing out a tolling project. It addresses material, financial, legal and environmental considerations. 

An example of an audit guide that focuses on environmental, health, and safety inspections during the toll-processing phase is included as Appendix B, Sample Toller HS E Assessment. This example should be customized to meet specific needs by adding quality concerns or other processing related items. 

Deliberate tampering is fraud or sabotage. Selective sampling can result in biased environmental measurements. Chemical analyses may be falsified to save time if the lab is overloaded with samples. Samples may be purposely put in metal containers to increase the measured metal amount. Intentional tampering can sometimes be identified by a trained eye during a routine audit. In other cases, surreptitious observation may be necessary. 

During each audit, at least 15% of the facilities operated, with a minimum of one facility, should be audited. The facilities included in the audit should not be the same as those included in the previous audit. When sufficient deficiencies are identified in the effectiveness of any safety and environmental management program elements, the test sample size shall be expanded for that program element. 

ANSI Z9.5-2003, Laboratory Ventilation, is intended for use by employers, architects, occupational and environmental health and safety professionals, and others concerned with the control of exposure to airborne contaminants. The book includes new chapters on performance tests, air cleaning, preventative maintenance, and work practices. It also highlights the standard s requirements and offers good practices for laboratories to follow. The book also offers referenced standards and publications, guidance on selecting laboratory stack designs, an audit form for ANSI Z9.5, and a sample table of contents for a laboratory ventilation management plan. 

It will also be necessary to consider environmental consequences at this time, and good neighbour policies. Use of checklists enables monitoring to be carried out during internal and external audits. A copy of each completed checklist should be held on file on projects for ease of reference. It Is Important that all checklists are signed off by project management when completed. Sample checklists are included (Figs 7.1 to 7.4).
