Drug withdrawal

Florfenicol (2) has been approved in Japan for the treatment of pseudo-tuberculosis caused by Pasteurellapiscicida and streptococcosis m. yeUowtail fish. The recommended dose is 10 mg/kg for up to one week and the drug withdrawal time is five days after cessation of treatment. Florfenicol is active in bovine respiratory disease caused by Pasteurella species and mastitis caused by Staphylococci and Streptococci. It is also effective in neonatal cohbacillosis caused by E. coli. The drug is being developed worldwide by Schering-Plough Animal Health for the treatment of aquatic and bovine diseases. 

Seizure disorders are generally categorized as idiopathic or acquired. Idiopathic seizures have no known cause acquired seizure disorders have a known cause, including high fever, electrolyte imbalances, uremia, hypoglycemia, hypoxia, brain tumors, and some drug withdrawal reactions. Once the cause is removed (if it can be removed), the seizures theoretically cease.

Low antiepileptic drug levels, drug overdose (e.g., cocaine, isoniazid, theophylline, phenothiazine), ethanol-related, and drug withdrawal... 

Restart medication (e.g., levodopa) if episode is believed to be a result of drug withdrawal... 

If it is necessary to use systemic corticosteroids for long-term control therapy, once-daily or every-other-day therapy should be used and repeated attempts should be made to decrease the dose or discontinue the drug. Withdrawal of chronic therapy may precipitate adrenal failure or unmask underlying inflammatory disorders such as Churg-Strauss syndrome. 

Describe the different approaches to treating drug withdrawal, and identify the circumstances in which each of these different approaches would be most appropriate. 

There is a wide range of treatment options provided by the social services and/or medical agencies. The most successful approaches are multi-component. They involve a psychosocial element to increase the appropriate life skills and a drug component to handle the immediate problems of drug withdrawal. Counselling and other behavioural therapies are useful components of most treatment programmes (Epstein et al., 2003). 

If drug withdrawal is planned, it should be done at least 6 months prior to conception.

It is important to be aware of possible adverse drug withdrawal events (ADWE). These events may be caused by physiological withdrawal reaction, but it is also possible that the underlying disease is worsened. An example of ADWE is delirium or seizures that may occur after abrupt discontinuing of benzodiazepines or alcohol.

Finally, a recent review [35] reported that QT prolongation (with or without TdP), together with hepatotoxicity, was responsible for more than 60% of drug withdrawals over the last 16 years.

Supporting this conclusion is the fact that approval times have decreased from 27 to 14 months. However, the GAO also reported a small increase in the drug-withdrawal rate since the implementation of PDUFA. That is, a higher percent of approved drugs has been withdrawn from the market because of safety issues. FDA officials argue that the increase is insignificant - from 3.10% in the 8-year period before PDUFA to 3.47% in the 8-year period after.
