Documentation, management program elements

For the first time, all the essential elements and components of a model of a technical management program in chemical process safety have been assembled in one document. We believe the Guidelines provide the umbrella under which all other CCPS Technical Guidelines will be promulgated. 

The basis for your program plan should be your team s assessment of the current PSM status (Figure 4-11), which will have yielded a list of deficiencies compared with the required PSM elements. These gaps must now be translated into statements of required tasks, which in turn suggest work products for inclusion in your implementation plan. For example, your assessment shows that the process knowledge and documentation management system needs improvement. To address this gap you need to plan a series of tasks. 

DOE 0 151.1A, Chapter IV, sec. 4b 4.4.5 A formal exercise program shall be established to validate all elements of the emergency management program over a multiyear period. Each exercise shall have specific objectives and shall be fully documented. Exercises shall be evaluated using an established critique process. Corrective actions shall be identified and incorporated into the program. 

Plan, implement, and manage all program elements discussed in API RP 75 (incorporated by reference) and to document the continued development, improvement, and success of SEMS plan. 

Management (owner and operator) should require that the program elements discussed in Sections 2 through 12 of this publication are properly documented and available at field and/or office iocations, as appropriate for each program element. 

Various elements in the safety and environmental management program identify documentation requirements. In addition to those requirements, documentation should be sufficient to describe the core elements of the program and the interaction between the elements. 

The first document produced under the CCPS program was a brochure entitled "A Challenge to Commitment," which was mailed to the CEOs of more than 1500 companies. It provides an overview and an outline of a comprehensive model for the technical management of chemical process safety, characterized by twelve distinct and essential elements. 

An active program of surveillance of the quality of the immunostains produced must be defined. The primary elements of such a quality assurance (QA) program include procedures and policies for patient test management, quality control, proficiency testing, comparison of test results, relationship of clinical information to patient test results, personnel assessment, communications, complaint investigations, QA review with staff, and QA records. The documentation and review by the laboratory director of all QA procedures is imperative and cannot be overstressed. A brief explanation of each of the QA elements is as follows ... 

Management interviews and the examination of program documents and records reveal whether the safety and loss prevention program contains essential elements with written standards of performance based on Dow s best global know-how.
