Finally, Shafran et al. [52] presented recently an open-label study on the efficacy and safety of rifaximin 600 mg/ day for 16 weeks in the treatment of mildly to moderately active CD. At the end of the study, 59% of patients were in remission (as defined by a Crohn s Disease Activity Index, CDAI, <150) with a significant reduction of the... [Pg.101]

Adults - Take 9 mg once daily in the morning for up to 8 weeks. Swallow capsules whole do not chew or break. For recurring episodes of active Crohn disease, a repeat 8-week course of budesonide can be given. Following an 8-week course of treatment for active disease and once the patient s symptoms are controlled (Crohn Disease Activity Index less than 150), budesonide 6 mg is recommended once daily for maintenance of clinical remission for up to 3 months. If symptom control is still maintained at 3 months, an attempt to taper to complete cessation is recommended. Continued treatment with budesonide 6 mg for more than 3 months has not been shown to provide substantial clinical benefit. [Pg.256]

NSAID-refractory-AS is defined when after treatment with at least 2 different NS AID over a period of at least 2 months, ASAS 20 is not obtained, and ESR, CRP, and the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) score do not improve, or worsens versus baseline. Indications for therapy with SBC-5-IMNs are in NSAID-refractory AS with ESR > 40 mm and a Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) > 4. Less than 5% of patients with NSAID-refractory AS in the community at large fulfill these conditions. Remission is defined when ESR and VAS have declined to <10 mm (men <5 mm) and the disease activity scores are <1 (scale 0-10). Remission with oral drugs is defined when remission is maintained with oral drugs for at least 2 years. Remission without drugs is defined when remission without drugs is sustained without relapse for at least 2 years. [Pg.666]

E. Therapeutic response The safety and efficacy of a single intravenous dose of Remicade were assessed in a randomized, double-blind, placebo-controlled study of patients with moderate to severe active Crohn s disease who had failed standard therapy. The primary end point was the proportion of patients who experienced a clinical response, defined as a minimum decrease in the Crohn s Disease Activity Index from baseline at the 4-week evaluation and without an increase in Crohn s... [Pg.298]

Based upon these observations and following Phase I safety studies, a Phase II randomized, double-blind, controlled clinical trial was undertaken to treat active ulcerative colitis with escalating doses of ISIS 2302. Based on an intent-to-treat analysis, 59% (13/22) of patients treated daily with 240 mg ISIS 2302 enema over six weeks achieved a positive response, as measured by Disease Activity Index (DAI)... [Pg.248]

In the study of SLE patients, we and other authors have found that there is a correlation between SLE disease activity index (SLEDAI) and the production of IL-16 (L7) and IL-18 (W19). It was suggested that IL-16 and IL-18 may be a useful indicator of disease activity of SLE. Plasma MCP-1 concentration has also been proposed as a marker for monitoring joint inflammation in rheumatoid arthritis (El). [Pg.31]

Other methods of clearing autoantibodies are extracorporeal and use the im-munoadsorption principle. Hollow fibers with dextran sulfate-coated cellulose membranes can adsorb pathogenic anti-DNA subgroups of high avidity and/or cationic antibodies, anticardiolipin, and anaphylatoxins (S35). The SLE disease activity index significantly decreased after such treatment (S35). However, the indication is still controversial. [Pg.154]

In a randomized, multicenter study in 94 patients, mesalazine 4 g/day for 12 weeks in a microgranular formulation was as effective as a standard dose of a glucocorticoid (6-methylpredisolone 40 mg/day) in mild to moderate Crohn s ileitis (Crohn s Disease Activity Index 180-350) (9). The group treated with methylpredisolone had a higher number of adverse events than those given mesalazine. The only adverse effect related to mesalazine was acute pancreatitis, which resolved on withdrawal. [Pg.138]

Systemic lupus erythematosus Single-center experience, double-blind, randomized, controlled trial 24 weeks Leflunomide 100 mg/day for 3 days, then 100 mg/day n = 6) versus placebo n = 6) Disease activity fell significantly in both groups after 6 months, and the reduction in SLE disease activity index from baseline to 24 weeks was significantly greater with leflunomide than with placebo (29)... [Pg.2018]

The severity of ulcerative colitis may be assessed by factors such as stool frequency, presence of blood in stool, fever, pulse, hemoglobin, erythrocyte sedimentation rate, C-reactive protein, abdominal tenderness, and radiologic or endoscopic findings. The severity of Crohn s disease can be assessed by the Crohn s disease activity index, which includes stool frequency, presence of blood in stool, endoscopic appearance, and physician s global assessment. [Pg.649]

The clinical significance of the degree of bowel wall thickening in known CD patients is controversial. Several studies attempted to establish a relationship between maximum bowel wall thickness and clinical (Crohn s disease activity index, CDAI) and biochemical (erythrocyte sedimentation rate, C reactive protein) parameters of CD activity. However, almost all the results of these studies produced weak corre-... [Pg.63]

Observational studies The efficacy of pulse pamidronate therapy in NSAID-refractory active ankylosing spondylitis has been assessed in 52 patients using the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) 47 completed the study [9 ]. They were given an NSAID and monthly intravenous infusions of pamidronate 60 mg for 6 months. The most common adverse reactions were arthralgia and myalgia, in 11 cases each, after the initial infusions. There were five withdrawals two defaults, one for a hip joint replacement, and two because of asymptomatic rises in serum creatinine. [Pg.787]

In an observational study, 95 patients with Crohn disease activity index <150 were randomly assigned to 6-mercaptopurine (0.5-1.5mg/kg/day, n=30), Elental as an elemental diet (>900kcal/day,... [Pg.598]
