The potassium-channel openers are thought to work by opening a subset of K -channels, which leads to membrane stabilization. Agents such as this have so far been proposed as vasodilators for the treatment of hypertension or heart failure, e.g. nicorandil. They may, however, become used for their smooth muscle relaxant action at other sites, e.g. in the aihvays as ANTIASTHMATIC AGENTS, or for bladder hyperexcitability. The most significant toxicity from diazoxide has been excessive hypotension, resulting from the recommendation to use a fixed dose of 300 mg in all patients. Such hypotension has resulted in stroke and myocardial infarction. The reflex sympathetic response can provoke angina, electrocardiographic evidence of ischemia, and cardiac failure in patients with ischemic heart disease, and diazoxide should be avoided in this situation. [Pg.237]
