High output left ventricular failure has been described after hysteroscopic lysis of adhesions using dextran as a distension medium. Prolonged surgical dissection of the uterine wall (the precise duration of the operation was not stated in the report) and the large volume of dextran and fluid (2 liters of 5% dextrose and an additional 800 ml of dextran) probably caused the dextran to enter into the systemic circulation, inducing a significant shift of fluid into the intravascular compartment (2).

Local reactions to intravenous iron dextran include transient pain (in some 4% of cases) and phlebitis. The risk of the latter may be reduced by using saline instead of 5% dextrose as diluent. Nevertheless, deep vein thrombosis has been observed in a few patients infused with a dilution of iron dextran in normal saline (SED-8, 514).

Total replacement doses of IV iron dextran have been given as a single dose, diluted in 250 to 1000 mL normal saline or 5% dextrose in water and infused over 4 to 6 hours. A test dose is still required. The ability to give a total dose infusion is a benefit of iron dextran over the other parenteral iron products. Iron dextran is best utilized when smaller frequent doses of sodium ferric gluconate or iron sucrose are impractical, such as with peritoneal dialysis. 

For example, 7.5% NaCI in 6% dextran-70 (hypertonic saline-dextrose) has been studied in a variety of animal models and in human trauma patients with some success. Particularly promising are animal studies of closed head injury, which suggest not only a hemodynamic benefit but also a sustained decrease in intracranial pressure. Studies in this system as well as in burn and trauma patients are ongoing.
