At the site of lesion, diverse reactive substances stimulate the endothelium to produce inflammatory cytokines, chemoattractants and other inflammatory mediators. The cytokine-activated endothelium expresses adhesion molecules that lead to the low affinity interactions between leukocytes and endothelium, which is mediated by selectins and described as rolling. Subsequently integrins mediate the firm adhesion of leukocytes, which allows emigration of the cells from venules into the interstitial compartment. Activated mast cells, PMNs and macrophages secrete cytokines (TNFa), lipid mediators (LTB4) and other inflammatory players (histamine, NO). 

The bead-based technology works not only for inflammatory mediators that have diagnostic/prognostic valne, bnt also for others (e.g., C-reactive protein, IL-6). This can make bead-based systems even more powerful. Kofoed et al. combined in-house and commercially available kits and used bead-based Luminex systems to assay biomarkers of potential interest in EDTA-plasma samples (70). A 3-plex assay for suPAR, sTREM-1, MIF was added to a commercially available human cytokine panel, IL-1P, IL-6, IL-8, GM-CSF, and TNF-a. Compared to healthy controls, all eight analytes were significantly higher in plasma from bacterial sepsis patients. 

Other inflammatory mediators, activated macrophages, T-lymphocytes, and mast cells also attach themselves to the endothelium and lead to the release of additional mediators, (adhesion molecules, cytokines, chemokines, growth factors), with important roles in atherogenesis. Also platelet s P-selectin induces TF and cytokine expression from monocytes (39).
