Immediate Aftermath of Trauma. We recommend instituting, whenever possible, a 10 day course of propranolol (10 mg three times daily) within hours of exposure to a severe traumatic stressor. Although the data supporting it remains limited, the brief duration of therapy coupled with the relative tolerability of propranolol supports use of this intervention. This medication therapy should, of course, be implemented in conjunction with appropriate crisis intervention services. Benzodiazepines should be avoided if possible during this critical window of vulnerability. 

DWl Unfortunately because resources have deteriorated and workers have far higher caseloads it s a lot of just crisis intervention. You know things get left and left and left until you re at the point where you know the kids are ready to come into care because nothing s been done prior to that. 

Part of ACM is educating patients and their families about the nature of their illness. Studies, however, have shown that education alone has no specific benefit in reducing clinical morbidity. Thus, ACM must include crisis intervention to realize a better quality of life and to avoid hospitalizations. 

Dr. T evaluates Mr. F for suicidality and implements a crisis intervention plan to ensure his safety. He meets with him for extended visits over the next 2 months, starts him on an SSRI, and refers him to Dr. E, a therapist who works in Dr. T s office. A general medical and cardiac evaluation is started as well. Over the course of 3 months, Mr. F requests and receives two more HFV tests, both of which are negative. Unfortunately, these negative results do little to weaken his belief that he has AIDS. On a scale of 0-100, Mr. F says he is 90% sure he has AIDS. 

Some policies may cover medications but not therapy, and other policies may do the opposite. In addition, patients may be willing to pay out of pocket for some services but not others. Limits or caps may be placed on medication charges or on the number of psychotherapy sessions allowed. Only certain types of providers may be covered by insurance, that is, only a psychiatrist or only certain types of situations, such as case management or crisis intervention only. Some services might require a primary care physician s referral, even when that physician knows very little about the patient or the problem. Finally, the deductible (the amount the patient must pay initially to obtain the service) can vary, depending on the insurance policy.

Psychoiogicai triage identifies those at greatest risk for psychiatric compiications. Crisis intervention and sociai support are key eiements of psychoiogicai first aid. Acute stress disorder (ASD) in the immediate aftermath increases risk for iafer posttraumatic stress disorder (PTSD).

Crisis intervention is still the mainstay of disaster counselors. Crisis intervention is a technique used to assist persons whose coping abilities have been overwhelmed by a stressful event. Most survivors at some point in the evolution of a disaster experience a level of stress so overwhelming that usual coping is inadequate to meet the need. Two key tools of the crisis/disaster worker are active listening and problem solving. 

Mobile outreach units provide information, clean needles and syringes, first aid, crisis intervention and services in locations where drug users meet or to specific target groups, such as sex workers. In France, Ireland and the Netherlands, such mobile units may also provide methadone.

Healthcare providers treat a person who is under the influence of LSD by using a talk-down approach where they talk the patient through the episode in a quiet, relaxed environment. If this approach fails, drug therapy is employed using a benzodiazepine such as diazepam (Valium). Drugs should only be used along with crisis intervention therapy. 

Evaluative research is needed on interventions for preventing or ameliorating adverse psychological effects in emergency workers, victims, and near-victims. Specific crisis intervention methods may be necessary for chemical or biological terrorist incidents, but in the absence of such incidents researchers might draw on studies of chemical spills, epidemics of infectious disease, and more conventional terrorist incidents.

If a violent incident occurs, the employer should provide the affected en loyees with emotional support such as crisis intervention and counseling.
