Stupor. A lowered level of consciousness manifested by the subject s responding only to vigorous stimulation. 

Generalized seizures include absence, myoclonic, and tonic-clonic. Manifestations of a generalized tonic-clonic seizure include alternate contraction (tonic phase) and relaxation (clonic phase) of muscles, a loss of consciousness, and abnormal behavior. Myoclonic seizures involve sudden, forceful contractions involving the musculature of the trunk, neck, and extremities. Absence seizures, previously referred to as petit mal seizures, are seizures characterized by a brief loss of consciousness during which physical activity ceases. The seizures typically last a few seconds, occur many times a day, and may go unnoticed by others. 

Qureshi et al." evaluated the timing of deterioration in patients with massive MCA strokes in a multicenter retrospective chart review. They found that 68% of patients manifested clinical deterioration by 48 hours, and nearly another 20% did so by 72 hours. Thus, the first 3-5 days appears to be the most crucial time for detecting patients at high risk for deterioration, although there was a small minority of patients who had deterioration at greater than 5 days from symptom onset. Early impairment in consciousness was also found to be predictive of mortality in one cohort of patients within a randomized chnical trial." Uremic and dialysis encephalopathies. Patients with renal failure continue to manifest neuropsychiatric symptoms despite significant advances in therapeutics and management.
