It may be argued that the patch tests are positive because the skin is in a state of hyper-reactivity termed excited-skin syndrome or angry back (Bruynzeel and Maibach 1995). This may lead to strong positive reactions to patch tests of minor irritants or subclinical or weak allergens. Authors who report compound allergies, such as Cox et al. (1989) or Kellett et al. The authors above have shown the need for particular ingredients to be simultaneously present, demonstrated the formation of allergenic reaction products, ruled out accidental contamination and reproduced the allergic reaction with their own pharmacy s preparations. Compound allergy is used to describe the condition in patients who are patch-test positive to formulated products, but are negative to all the ingredients tested individually. This phenomenon can sometimes be explained by irritancy of the original formulation, but, in some cases, it has been demonstrated that reactivity was due to combination of the ingredients to form reaction products (Bashir and Maibach 1997). [Pg.373]
