Colonic biopsies

A 52-year-old man developed watery diarrhea 6-8 times a day 2 weeks after he had started to take acarbose 100 mg. In 3 weeks he lost 3 kg. Duodenal biopsies were normal colon biopsies showed a large increase in intraepithelial lymphocytes. The mononuclear cells expressed CD-25, and HLA-DR antigen was increased in epithelial cells. Within 4 days of acarbose withdrawal the diarrhea had disappeared, and biopsies 4 months later showed that CD-25 expression in the cells of the lamina propria was improved and HLA-DR was no longer expressed by the epithelial cells. On rechallenge the diarrhea recurred within 3 days. Biopsies showed pronounced HLA-DR in the epithelial cells and CD-25 expression in some mononuclear cells in the lamina propria. 

A 41-year-old woman had diarrhea, fever, and a skin rash after taking carbamazepine 300 mg/day for 4 weeks. Colitis was confirmed at colonic biopsy. 

In a few reports, lymphocytic colitis has been attributed to ticlopidine in some cases the pathological features in the colon biopsy resolved after withdrawal of the drug (24-26).

All colonic biopsies showed erosions with severe infiltration by inflammatory cells. Masson s trichrome staining identified subepithelial collagen bands as thick as 25 pm in every specimen, consistent with collagenous colitis. Lansoprazole was withdrawn, and within a few days the watery diarrhea disappeared. Colonoscopy 2 months later showed a reduction in the number of red spots. Biopsies from the cecum, ascending, transverse, and descending colons, and rectum, showed no collagen bands.

In a retrospective study, among 43 colonic biopsies from renal transplant recipients with MMF administration and persistent diarrhoea, 33 cases showed MMF-related colitis. Crypt distortion and loss as well as increased apoptosis constituted the main features [35 ].
