Kearns-Sayre syndrome [17] A multisystem disorder characterized by the invariant triad onset before age 20 years, PEO, pigmentary retinal degeneration plus at least one of the following complete heart block, cerebrospinal fluid protein above 100 mg/dl, cerebellar ataxia. Large-scale heteroplasmic mitochondrial DNA deletions are frequently detected in skeletal muscle (rarely in other tissues). 

Glucocorticoids given intrathecally can cause a rise in cerebrospinal fluid protein and carry the risk of arachnoiditis (SED-8, 820). Chemical meningitis has been reported after two intrathecal injections of methylpredni-solone acetate (450) and after lumbar facet joint block (SEDA-17, 450). Intraspinal injections of hydrocortisone for multiple sclerosis apparently led in one case to a cauda equina syndrome, with subsequent ulceromutilating acro-pathy (SEDA-17, 450). Intra-discal injections of triamcinolone acetonide in a number of French cases led to disk or epidural calcification, sometimes symptomless (SEDA-17, 450).

Mean cerebrospinal fluid concentrations were estimated to be 0.3 0.2 (ig/ L in patients whose mean plasma concentrations were 76.9 34 (ig/ L [8], This indicates that nimodipine does not apparently bind to the cerebrospinal fluid proteins. The bioavailability of dihydropyridines following oral administration was found to increase after food intake, such as grapefruit juice [9]. After intravenous administration, nimodipine is distributed rapidly and uniformly. On the other hand, after oral administration, nimodipine is strongly differentiated, with high concentrations being found in the liver, kidneys, and fat, low concentrations in the brain and testes, and the rest in the plasma [10, 11]. Only small amounts of nimodipine pass through the placenta [11].
