Cardiotoxic effects are relatively uncommon with mianserin (2). In a placebo-controlled study in 50 patients with a variety of cardiac conditions who were taking anticoagulants, mianserin (up to 30 or 60 mg) had no effects on electrocardiography, blood pressure, or pulse rate after 3 weeks. In a second phase, mianserin (up to 60 mg/day) was compared with amitriptyline (up to 150 mg/day) and placebo in 18 healthy volunteers. Measurements included systolic time intervals, electrocardiography at rest and during exercise, echocardiography, and blood pressure. Amitriptyline had a negative inotropic effect mianserin increased ejection fraction. The results of both these experiments led the authors to conclude that mianserin is an antidepressant with very low cardiac toxicity. 

Prolonged QT interval and symptomatic ventricular tachycardia has been described in a patient who took loratadine plus quinidine, but is most likely to have been attributable to the quinidine and the patient s cardiac condition (SEDA-19,174). 

Chronic overdosing is more likely to cause thyrotoxicosis than an acute overdose. Thyrotoxicosis should be suspected in patients exhibiting tachycardia, cardiac arrhythmias, hypertension, tremors, and seizures. Coma and circulatory collapse can be seen in severe cases of thyrotoxicosis. This is especially dangerous in patients with cardiac conditions. Deaths have also occurred in healthy adults that have used thyroid extract to lose weight. 

The changes of serum CK and its MB isoenzyme following a myocardial infarction are discussed in Chapter 44. Other cardiac conditions have been reported to increase serum CK and CK-MB in serum. These conditions include cardioversion, cardiopulmonary bypass and coronary artery bypass surgery, cardiac transplantation, myocarditis, pericarditis, and pulmonary embolism. Despite improvements, in the diagnostic performance and practicality of CK and CK-MB assays, there is no controlled cUnicai impact trial showing that these tests are effective for decisions to send a patient home or to the appropriate level of care of admission for patients with suspected acute cardiac ischemia, either as one-time or serial tests.For diagnosis of acute myocardial infarction, it is now advantageous to use more cardiac-specific nonenzymatic tests, such as cardiac troponin I orT.
