In the past, the acid infusion or Bernstein test was used to confirm the diagnosis, but for the most part, this has proven to be relatively insensitive and has been superseded by the use of esophageal pH monitoring. Once the diagnosis has been confirmed, treatment with PPIs has been shown to be effective in up to 85% of patients. In some centers, individuals have undergone fundoplication, with comparable resolution of symptomatology. 

The preceding treatment relates primarily to flocculation rates, while the irreversible aging of emulsions involves the coalescence of droplets, the prelude to which is the thinning of the liquid film separating the droplets. Similar theories were developed by Spielman [54] and by Honig and co-workers [55], which added hydrodynamic considerations to basic DLVO theory. A successful experimental test of these equations was made by Bernstein and co-workers [56] (see also Ref. 57). Coalescence leads eventually to separation of bulk oil phase, and a practical measure of emulsion stability is the rate of increase of the volume of this phase, V, as a function of time. The first attempt to prepare an azaindole as an azalog of biologically active compounds appears to have been made by Bernstein et al. They synthesized 6-amino-2,3-diphenyl-7-azaindole and tested it for antimalarial activity against Plasmodium lophurae. It showed little activity.

Ojala etal. (1998) reported on the crystallization of two conformational polymorphs (Bernstein 1987) (see Chapter 5) of acetone tosyUiydrazone 3-XX. A triclinic form and a monoclinic form are both obtained from anhydrous ethanol—sometimes together. Sulphonechloramide (chloramine-T), an antibacterial used in the food and brewing industry, has been shown to induce IgE-mediated OA which can be detected by skin testing using this agent alone without conjugation to a protein carrier (Malo and Bernstein, 1993).

Positive patch tests to phenolphthalein have been reported in some eczematous lesions (Bernstein 1931 Wise and Sulzberger 1933). False positive patch tests, reflecting epidermal hysteresis, have also been demonstrated after phenolphthalein administration (Shelley et al. 1972). Wyatt et al. (1972) found that serum taken during exacerbations produced a local inflammation when injected intradermally. Possible cross-reaction with erythrosin which is used as a food and drug colorant has been reported (Wile 1936). Other rare allergic manifestations after phenolphthalein administration are urticaria, Stevens-Johnson syndrome, lupus erythe-matosus-like eruptions, and encephalitis (Lindemayr 1959 Kendall 1954).
