Phenothiazines Barbiturates

Phenazocine is administered intramuscularly in doses of 1 to 3 mg every 4 to 6 h. Its depressant effects are potentiated by CNS depressants such as the barbiturates, phenothiazines, and anesthetics so that smaller doses are indicated when these are used concomitantly with the phenazocine. 

In New 2 ealand, the 1984 Medicines Regulations require that the 6 types of medicines that were most frequently associated with potentially serious poisoning incidents in children be supplied in strip or blister packaging which is reasonably resistant to attempts of young children to gain access to contents. These medications arc aspirin, paracetamol, iron salts, barbiturates, phenothiazines, and antidepressants. Liquid forms of those medicines may be dispensed with a container fitted with a CRC. It should be noted that this list is now somewhat dated, given that barbiturates are not now generally available in New Zealand. 

The stability constants K(.) have been determined by h.p.l.c. for 1 1 inclusion complexes of cyclohexa- and cyclohepta-amyloses with barbiturates, phenothiazines, sulphonylamides, and sulphonylureas. Results were in good agreement with those obtained by solubility and spectroscopic methods. Use of h.p.l.c. is particularly suitable for chemically unstable compounds like pheno-thiazine.

Allopurinol, barbiturates, benzodiazepines, captopril, carbamazepine, erythromycin, fluoroquinolones, isoniazid, NSAIDs, penicillins, phenothiazines, phenytoin, rifampin, sulfonamides antimicrobials, and tetracyclines... 

Drugs that may affect repaglinide include CYP 450 inhibitors (eg, clarithromycin, erythromycin, ketoconazole, miconazole), CYP 450 inducers (eg, barbiturates, carbamazepine, rifampin), beta blockers, calcium channel blockers, chloramphenicol, corticosteroids, coumarins, estrogens, gemfibrozil, isoniazid, itraconazole, levonorgestrel and ethinyl estradiol, MAOIs, nicotinic acid, NSAIDs, oral contraceptives, phenothiazines, phenytoin, probenecid, salicylates, simvastatin, sulfonamides, sympathomimetics, thiazides and other diuretics, and thyroid products. 

Exercise caution when giving the drug with alcohol and other CNS-acting agents such as phenothiazines, barbiturates, and belladonna derivatives. 

Drugs that may affect trazodone include carbamazepine, phenothiazines, and venlafaxine. Drugs that may be affected by trazodone include alcohol, barbiturates, CNS depressants, digoxin, MAOIs, phenytoin, and warfarin.

Certain concomitantly administered drugs may interfere with the effectiveness of the oral contraceptives or lead to an increased incidence of breakthrough bleeding. These include rifampin, isoniazid, ampiciUin, neomycin, penicillin V, chloramphenicol, sulfonamides, nitrofurantoin, phenytoin, barbiturates, primidone, analgesics, and phenothiazines. 

Estazolam potentiates the CNS depressant effects of phenothiazines, narcotics, antihistamines, MAOIs, barbiturates, alcohol, general anesthetics, and TCAs. Use with cimetidine, disulfiram, oral contraceptives, and isoniazid may diminish hepatic metabolism and result in increased plasma concentrations of estazolam and increased CNS depressant effects. Fleavy smoking (more than 20 cigarettes/day) accelerates estazolam s clearance. Theophylline antagonizes estazolam s pharmacological effects. 

Triazolam potentiates the CNS depressant effects of phenothiazines, narcotics, antihistamines, MAOIs, barbiturates, alcohol, general anesthetics, and antidepressants. Use with cimetidine and disulfiram may increase triazolam s plasma concentration. 

The use of phenothiazines for acute schizophrenia is preferred because of several advantages they afford. They cause little sedation and the patient is therefore easily aroused.
