Many examples of the effects of tablet excipients on dmg decompositions are reported in the pharmaceutical literature. Chemical interaction between components in solid dosage forms may lead to increased decomposition. Replacement of the phenacetin in compound codeine and ARC tablets by paracetamol in NHS formulations in Australia in the 1960s (because of the undesirable side-effects of phenacetin), led to an unexpected decreased stability of the tablets. The cause was later attributed to a transacetylation reaction between aspirin and paracetamol and also a possible direct hydrolysis of the paracetamol (Scheme 4.15).

Toxicity in industry does not exist [3,4]. All lethal intoxications are attributed to therapeutic uses of bismuth compounds, mostly injections or long-lasting treatments. Reports in the 1970s from Australia and France about serious intoxications induced by bismuth led to an important reduction of its medical use. In the 1980s some bismuth compounds were successfully tested in the treatment of chronic gastritis without the former side effects. It was previously used as a tranquilliser however, the much higher doses required often resulted in depression and Parkinson disease-like symptoms (Huang 1993). As a result Rauwolfia has become a restricted herb in Australia and has no place in current herbal prescribing.
