The immediate objectives are relief of pain and initiation of treatment demonstrated to reduce mortality. Subsequent management of proven myocardial infarction is concerned with treatment of complications, arrhythmias, heart failure and thromboemboli, and then secondary prevention of further myocardial infarctions. 

To reduce mortality, administration of an aldosterone antagonist, either eplerenone or spironolactone, should be considered within the first 2 weeks following MI in all patients who are already receiving an ACE inhibitor (or ARB) and have an EF of equal to or less than 40% and either heart failure symptoms or diagnosis of diabetes mellitus.3 Aldosterone plays an important role in heart failure and in MI because it promotes vascular and myocardial fibrosis, endothelial dysfunction, hypertension, left ventricular hypertrophy, sodium retention, potassium and magnesium loss, and arrhythmias. Aldosterone antagonists have been shown in experimental and human studies to attenuate these adverse effects.70 Spironolactone decreases all-cause mortality in patients with stable, severe heart failure.71... 

The short-term goals of therapy for IHD are to reduce or prevent anginal symptoms that limit exercise capability and impair quality of life. Longterm goals are to prevent CHD events such as MI, arrhythmias, and heart failure and to extend the patient s life.

During heart failure (HF), functional expression of different proteins involved in E-C coupling is altered, and these changes contribute to altered Ca transients, contractility and arrhythmias in HF. Diuretics should be avoided in the treatment of hypertensive diabetics or patients with hyperlipidemia. 

The Danish Investigations of Arrhythmia and Mortality ON Dofetilide (DIAMOND) study comprised two studies in patients at high risk of sudden death one in patients with congestive heart failure and one in patients with acute myocardial infarction within the previous 7 days (50). 

It is most important to consider the indication. Some disease conditions, such as diabetes, congestive heart failure, and nephropathy predispose for arrhythmias. In a case when low level of hERG inhibition with an estimated 30 TI caused no prolongation of QT in healthy dogs and consequently in healthy volunteers, unexpected QT prolongation occurred when the drug was administered to patients with type 2 diabetes. Advances in ryanodine receptor (RyR2)/calcium release channel research have led to the development of novel compounds targeting RyR2 for treating heart failure and for preventing lethal arrhythmias.
