Cefazolin or cefuroxime are appropriate for prophylaxis in cardiothoracic and vascular surgeries. In the case of 3-lactam allergy, vancomycin or clindamycin are advised. Debate exists on the duration of antimicrobial prophylaxis. The National Surgical Infection Prevention Project cites data that extending prophylaxis beyond 24 hours does not decrease SSI rates and may increase bacterial resistance.1 American Society of Health-System Pharmacists guidelines from 1999 allow for the continuation of prophylaxis for up to 72 hours.22 Duration of therapy should be based on patient factors and risk of development of an SSI. SSIs are rare after cardiothoracic operations, but the potentially devastating consequences lead some clinicians to support longer periods of prophylaxis. [Pg.1236]

The agents like protamine sulfate react with the strongly acidic groups of heparin and can abolish its anticoagulant activity. Approximately 1 mg of protamine sulfate neutralizes 80 to 100 units of heparin. It is used only in severe bleeding or when heparin action needs to be terminated rapidly e.g. after cardiac or vascular surgery. [Pg.245]

For some reactions the usefulness of withdrawing metformin before operations, except in emergency, cardiac, and vascular surgery and in operations requiring deliberate hypotension, has been discussed (37) and the creatinine threshold value has been discussed (38). [Pg.370]

Besides the promising use of BASYC in experimental vascular surgery we see the possibility for the application of BC as a soft-tissue substitution material in various medical fields because of its extraordinary properties. [Pg.80]

Men aged over 50 years with established CAD have an ED incidence of 40% and in those post myocardial infarction or post vascular surgery the incidence ranges from 39% to 64%, depending on diagnostic criteria (15). [Pg.506]

Due to the very low event rates in patients with asymptomatic lesions of moderate severity (<60% diameter stenosis), it is unknown whether currently available interventional techniques can improve long-term outcomes over those achievable with optimal medical management. Also unresolved are the indications for carotid stenting in asymptomatic individuals with contralateral carotid occlusion (53) and those undergoing major cardiac or vascular surgery (54). [Pg.558]
