Testicular neoplasm

The application of these principles is well illustrated in the current approach to the treatment of acute leukemia, lymphomas, Wilms tumor, and testicular neoplasms. 

Patients with seminomas are usually aged between 20 and 50, accounting for 40% of testicular neoplasm. 

In chronic inhalation studies rats and mice were exposed to 0, 75, 250, or 750ppm 6 hours/day, 5 days/week for 104 weeks. Eor male rats exposed at 750 ppm survival was decreased, and the incidence of renal tubule neoplasms and testicular adenomas was increased. The findings from an extended evaluation of the kidneys showed a significant increase in the incidences of renal tubule adenoma and hyperplasia in high-dose males and females. The existence of a barrier between the blood and testes is indicated by the absence of staining in testicular tissue after the intravascular injection of dyes. Morphological studies indicate that the barrier lies beyond the capillary endothelial cells and is most likely to be found at the specialized Sertoli-Sertoli cell junction. It appears that Pgp, the efflux transporter protein, also plays a role in forming this blood-testis barrier. This protein probably plays a role in preventing certain chemotherapeutic agents from reaching specific areas of the testis and thus hinders treatment of the neoplasm.

Cisplatin, in combination with bleomycin, etoposide, ifosfamide, or vinblastine, cures 90% of patients with testicular cancer. Used with paclitaxel, cisplatin induces complete response in most patients with carcinoma of the ovary. Cisplatin produces responses in cancers of the bladder, head and neck, cervix, and endometrium all forms of carcinoma of the lung anal and rectal carcinomas and neoplasms of childhood. The drug sensitizes cells to radiation therapy and enhances control of locally advanced lung, esophageal, and head and neck tumors when given with irradiation. 

A few years ago significant palliative results were obtained by chemotherapy in a number of human neoplasma. Today it is, however, possible to list at least certain neoplastic diseases that can be associated with a normal life expectancy after treatment with drugs alone or in combination with other modalities. These neoplasms essentially include carcinoma in women, acute leukemia, Burkitt s lymphoma, Ewing s sarcoma, retinoblastoma in children, lymphosarcoma, Hodgkin s disease, rhabdomyosarcoma, mycosis ftmgoides and testicular carcinoma. 

As on data nearly ten diflfemet types of neoplasms may be cured with the aid of chemotherapy in patients quite satisfactorily, namely leukemia in children, Hodgakin s disease, Burkitt s lymphoma, Ewing s sarcoma, choriocarcinoma in women, lymphosarcoma, mycosis fimgoides, rhabdomyosarcoma, testicular carcinoma, and retinoblastoma in children.
