Lindsey engendered other legends. Once, when a doctor asked some volunteers to enter a chamber filled with CS tear gas, Lindsey went right along with them and refused to come out. He found that if he could manage to tolerate the first twenty minutes or so, he would became adapted to the irritant effects and could function normally. The first polymeric sustained release product to reach the market was Ocusert , a pilocarpin sustained release ocular implant developed by Alza. Ocusert has the drug reservoir as a thin disc of pilocarpine-alginate complex sandwiched between two transparent discs of microporous membrane fabricated from ethylene-vinyl acetate copolymer. The microporous membranes permit the tear fluid to penetrate into the drug reservoir compartment to dissolve pilocarpine from the complex. Pilocarpine molecules are then released at a constant rate of 20 or 40 pg/hr for a four- to seven-day management of glaucoma. [Pg.353]

Contact Lens Fitting and Management Fluorescein staining of the tear film is a major aid in the fitting of rigid gas-permeable contact lenses.After topical application of fluorescein to the eye the tear layer becomes visible, with a characteristic pattern of green fluorescence. Tear duct fibrosis develops in one of six patients with excessive lacrimation from fluorouracil (66).
