In contrast to anticonvulsants and alcohol, drugs such as bupropion, fluoxetine, fluvoxamine, nefazodone, quinidine, paroxetine, and some antipsychotics can inhibit specific CYP enzymes (7, 11, 36, 37, 41, 42, 43 and 44). Thus, TCAs, certain BZDs, bupropion, some steroids, and antipsychotics can all have their metabolism inhibited by drugs such as fluoxetine. For example, fluoxetine at 20 mg/day produces on average a 500% increase in the levels of coprescribed drugs which are principally dependent on CYP 2D6 for their clearance. That can lead to serious or even life-threatening toxicity if the drug has a narrow therapeutic index and the dose is not adjusted for the change in clearance caused by the coadministration of fluoxetine. [Pg.37]

Estriol (estra-1,3,5(10)-trien-3,16-a,17-8 -triol) is the major estrogen metabolite found in the urine. It is excreted in the form of its conjugate with glucuronic acid. The determination of this steroid as an index of placental function has become one of the most widely used endocrine determinations. As pregnancy progresses, the excretion increases and reaches very high levels near term. In abnormal fetoplacental function, the levels of estriol will fall in some cases. The fall is usually progressive, and, because of this, serial determinations of urinary estriol must be carried out. The drop in estriol can be taken as evidence of placental insufficiency, and close watch by the physician is indicated, as a continued drop may necessitate Cesarean section to save the life of the infant. [Pg.499]

I7(x-Methyl-17 -hydroxy-estra-4,9,l l-trien-3-one (Methyltrienolone, N-99). In the course of a steroid total synthesis [293] this compound (Methyltrienolone, N-99) was prepared it was reported [55] to possess 6000% of the androgenic (ventral prostate index), 7500% of the androgenic (seminal vesicles index), and 12,000% of the anabolic (levator ani index) activity of methyltestosterone. Later the anabolic activity was reported [74] to be 30,000% of that of methyltestosterone. As measured by multiple parameters methyltrienolone turned out to be the most hepatotoxic steroid, causing biochemical symptoms of intrahepatic cholestasis [75]. It was also reported [74] to reduce the excretion of 17-ketosteroids and 17-hydroxycorticosteroids and to cause enhancement of the blood coagulation factors V, VII, and X. It also increases the prothrombin content of the plasma [74]. [Pg.88]
