The total gravity based penalty in this hypothetical can be reduced by a factor of 50 percent for voluntary self reporting within thirty days of discovery, another 15 percent for mitigating the violation by fifing a PMN and entering into a 5(e) order, and another 15 percent for a cooperative attitude. 

In addition to the CIR process the cosmetic industry has instituted a second, important, self-regulatory procedure the voluntary reporting of adverse reactions, which is intended to provide data on the type and incidence of adverse reactions noted by consumers or by their medical advisors. This reporting procedure creates early awareness of problems handled outside hospital emergency facilities or centers for acute poisoning. 

One of the key self-regulatory procedures in the cosmetic industry is the voluntary reporting process of adverse reactions. The program is intended to provide data on the type and frequency of adverse reactions reported by consumers or by their medical advisors to the industry. It is an important means of detecting problems that are not treated in hospital emergency rooms (i.e., documented in the National Electronic Injury Surveillance System, NEISS) or do not reach poison control centers. 

Accreditation is a voluntary process on the part of an institution. The steps in the accreditation process are the engineering program performs a self-study, a team of outside ABET evaluators then visit campus, conduct interviews and compare the program with established criteria, and a final report and finding are returned back to the institution for their review. 

While some of the systems described above involve written forms, safety and health employee program managers and safety professionals or other appropriate individuals should feel free to design their own hazard reporting forms tailored to their worksite and employees. The flexibility in design can allow for voluntary anonymity by making employee self-identiflcation optional. A box for the forms should be installed along with a specific location for the forms.
