For four years, I have been actively involved in The Giles Herald, a weekly campus newspaper. As the current managing editor, it is my responsibility to turn a 75-person staff into a high-performance team to produce 28 pages of news, sports, opinion, and arts each week. I have also been an active member of the debate team, an activity that has helped me develop excellent speaking and communication skills.

The required notification must be provided at ieast annually In writing. Acceptable forms of notice are, for example, a letter, product labeling, and product literature distributed to customers. If you are required to prepare and distribute a Material Safety Data Sheet (MSDS) for the mixture under the Occupational Safety and Health Act (OSHA) Hazard Communication Standard, your section 313 notification may be attached to the MSDS or the MSDS may be modified to include the required information. I am sending you a sample letter that you can use to request permission to use these figures. Usually if a request is written on the author s letterhead, the publisher responds more quickly. So you might request that Dr. Johnson assist you in this task. I am also attaching a copy of our copyright policy.

Submit Letter of Nonrepudiation Agreement. A letter of nonrepudiation agreement must be submitted to the FDA. See Appendix B, Sample Letters of Nonrepudiation Agreement, for letter examples. 

As obvious and unnecessary as this advice may sound, it is not rare to see a note that is too informal for the task. Sample letter 1 is such a letter (see Figure 9.1). It is too short, too folksy, and too familiar, with the therapist referring to both the patient and the doctor on a first-name basis. Also, it refers to another patient in the same letter. If the patient reads it, he or she would probably see it as something written by a friend rather than a therapist. If it were ever brought to court for any reason, it would seriously undermine the credibility of the therapist in the eyes of a judge and jury. 

Sample letter 3 (see Figure 9.3) is written to a psychiatrist. The first paragraph tells how long and for how many sessions the therapist has seen the patient and what psychotherapeutic approach has been used. It also says in the first paragraph that this is a psychopharmacology con-... 

Sample letter 4 (see Figure 9.4) is written to a mental health care professional who is acting as the screening clinician in the psychiatric emergency department of the county mental hospital. There is no mention of a release of information or consent from the patient, allowing the therapist and screening clinician to speak to each other. It would be good...
