Safety systematic record review

Systematic record keeping of all hazards and their impact including those related to health and safety hazards. This systematic record must also include all uncertainty, assumptions, and hazard screening if any. These are necessary for further reviews. 

The safety of essential oils is always an important issue. They are potent chemicals and should be handled correctly. Their properties, toxicity and contraindications are addressed in a number of sources and textbooks that highlight safety issues in a systematic and responsible way. Safety is always under review and findings are published and disseminated. There have been scare stories in the media and there are individuals whose raison d etre is to highlight the dangers of essential oils. They emphasize hazardous properties and concerns about certain components. There are essential oils that are best avoided but the vast majority of those used for aromatherapy have a good safety record. 

Systematic reviews Adverse reactions to Ciwujia injection (Siberian Ginseng injection) recorded in a major Chinese database have been reviewed [19 ]. Of 800 clinical studies on the efficacy and safety of Ciwujia injection, 97 (12%) reported 285 cases of adverse reactions, the most serious of which involved anaphylactic shock, with four deaths. The most common reaction reported in clinical studies was pain at the injection site. 

Death Data on the efficacy of buflomedil in intermittent claudication were retrieved from a Cochrane systematic review, and data on safety were obtained by comparing the number of reports of serious adverse events and deaths published in the literature with those reported in postmarketing surveillance databases The authors concluded that the evidence that buflomedil is beneficial is undermined by documented publication bias. They also concluded that there is bias in reporting adverse events to international safety databases, illustrated by the discrepancy between the number of published drug-related deaths (20), deaths recorded in the WHO database that were potentially drug related (20), and deaths that were attributed to buflomedil in the database of the international marketing authorization holder (11).

OSHA-recordable incidents are always investigated, and effective prevention is implemented. Reports and recommendations are available to employees. Quality and completeness of investigations are systematically reviewed by trained safety personnel. ... 

The term "coach" is very familiar to us in an athletic context. In fact, winning coaches practice the basic observation and feedback processes needed for effective safety coaching. They follow most of the guidelines reviewed here. As illustrated in Figure 12.4, the most effective team coaches observe the ongoing behaviors of individual players and record their observations in systematic fashion, using a team roster, behavioral checklist, or videotape.
