Renal tumors clear-cell

Carcinomas that are almost invariably negative but may occasionally show rare CK7-positive cells include hepatocellular carcinomas, duodenal ampullary carcinomas, colon carcinomas, renal (clear cell type), prostate, and adrenal cortical tumors. 

Metastatic renal cell carcinoma can also present with no history of a primary renal cell carcinoma. Histologically, these tumors usually exhibit clear and oncocytic cells with the fine vascular network of typical renal cell carcinoma. The differential diagnosis depends on the anatomic location but includes oncocytic tumors and tumors with clear cell differentiation such as clear cell hyalinizing carcinoma and clear cell myoepithelioma. Again, the immunostaining panel can help to differentiate these entities. 

The pathologic distinction between primary sebaceous carcinoma and metastatic renal cell carcinoma (RCC an important differential diagnostic alternative with a clear-cell appearance) is similar to that made earlier in reference to sweat gland carcinomas. Slow evolution of a solitary skin tumor favors a primary lesion. In addition, immunostaining for podoplanin is effective in this context that determinant is consistently present in sebaceous tumors, whereas it is absent in RCC. CD 10 is potentially seen in both lesional categories. TSS...

Von Hippel-Lindau disease predisposes to the development of various tumors (hemangioblastomas of the neuraxis and retina, tumors of the membranous labyrinth, renal clear cell carcinomas or cysts, pheochromocytomas, pancreatic cysts or tumors, and epididymal cystadenomas). Renal cancer constitutes one of the main causes of death (Richard et al. 1998). Conversely, meta-analysis of cases of coincident tuberous sclerosis complex shows that the risk of malignancy is comparable with the normal population (Tello et al. 1998). 

Bone metastases associated to renal neoplasm are suggestive of clear cell sarcoma. Rhabdoid tumor affects young children, and hypercalcemia and metastases at diagnosis are frequent in this highly aggressive tumor. Juvenile renal cell carcinoma occurs in the second decade and is frequently calcified and smaller than WT. Renal medullary carcinoma occurs in black adolescents with sickle cell trait involvement of the renal sinus results in caliectasis. Renal lymphoma is usually bilateral and associated with other sites of involvement.
