There is an important philosophical point here that must not go unremarked. We see that the sloping landscape of atomic diameter, with its occasional gentle rises, is the outcome of a competition of finely balanced forces. The CTDs were implemented in July 2003. They are format-based documents for submission to the regulatory authorities the country-specific process of review, for example, via the IND and NDA of the United States or the Centralized Procedure of the EMEA, is not affected. The harmonized CTDs help to reduce cost and accelerate approval time. Figure 7.1 shows the CTD structure five modules with Module 1 for regional administrative information specihc to each country. Module I is specific to the region in which the application is made (the EU in this case) and is, technically, not part of the CTD. It contains regional administrative information, a submission table of contents, the SmPC (i.e. the draft package insert), the patient information leaflet (PIL) (if any) and translation of the labeling into all relevant languages (in the case of a CP application this is now required in 22 languages). 

The role of interferon in advanced disease is even more unclear, especially for those patients who have recurred after treatment with adjuvant interferon therapy. Interferon-a has been used as a single agent in patients with metastatic disease who have not received adjuvant therapy, and in combination with chemotherapy and/or other biotherapy for metastatic melanoma. The challenges of combination therapy are that many of the toxicities seen with interferon can be exacerbated by concomitant chemotherapy (e.g., nausea, vomiting, and neutropenia). In an attempt to limit systemic toxicity and to potentate local benefits, the regional administration has been evaluated in a variety of settings. Intralesional and perilesional application of interferon has been shown to have some efficacy in small lesions and appears to be well tolerated. ...
