Recreating the Incident

When additional information is required or when some facts must be verified, you may need to re-create the incident. One caution if this recreation takes place, you must make sure that the investigation techniques do not result in another incident. 

Experienced individuals should conduct interviews. In some cases, the team assigned should inelude an individnal from legal or human resources. The following are reeommended steps in interviewing  

After interviewing all witnesses, the team should analyze each statement. In some cases, the team may wish to re-interview one or more witnesses to confirm or verify key points. While there may be inconsistencies in statements, investigators should assemble the available testimony into a logical order. Analyze this information along with data from the incident site.

Pictures should be taken of the accident scene. K contributory causes of the accident included site conditions, some pictures of those conditions should be taken. People reviewing the report should understand what the conditions were that contributed to the incident. A picture can help recreate the incident scene.
