Quality improvement opportunity identification

The quality improvement project or action starts with the identification of an improvement opportunity which is based on measures of quality losses and/or on comparisons against organizations recognized as leaders in a particular field. The opportunity for improvement is defined and the process involved is evaluated by analyzing data and facts. The objectives for improvement are introduced. 

Focused or targeted medication use evaluation follows a reasonably well-established cycle identification of a potential problem in the use of a specific drug or therapy, collection and comparison of data, determination of compliance with a pre-established guideline/expectation, and action as needed to improve discrepancies between expected and measured results. This type of medication use evaluation provides an excellent opportunity to apply the Shewhart cycle for continuous quality improvement (Figure 26.3). Focused medication use projects are typically selected for a specific reason. Table 26.4 lists reasons to consider drugs for focused evaluation projects. 

While in the recent past it was extremely difficult, time consuming and labour intensive to build such a library from purified natural products, with the advent of newer and improved technologies related to separation, isolation and identification of natural products the situation has improved remarkably. Now, it is possible to build a high quality and chemically diverse natural product library that can be suitable for any modern HTS programmes. Natural product libraries can also be of crude extracts, chromatographic fractions or semi-purified compounds. However, the best result can be obtained from a fully identified pure natural product library as it provides scientists with the opportunity to handle the lead rapidly for further developmental work, e.g. total or partial synthesis, dealing with formulation factors, in vivo assays and clinical trials. 

Field return data can be used for variety of purposes that include the prediction of future claims, the estimation of field reliability, and the identification of opportunities for quality and reliability improvement. (O Connor Kleyner, 2011). If one assumes that the return data is error free then it is not difficult to claim that it is the most trustworthy data in analyzing reliability of a product compared to accelerated test or simulation based data. However, in reality field return data contains errors corresponding to improper, correlated, incomplete, and poorly collected data that result in misleading reliability predictions.
